Digital transformation is typically a deliberate planned process by standard organizational (e.g. business) units to move over a period of time from one system (application, platform, etc) -- possibly manual or legacy computerization to a modern digital alternative or environment (e.g. SaaS/PaaS/IaaS cloud, etc.). Digital innovation is often a quick application of a new idea, usually unplanned and by a rogue operative, guerrilla group or evangelists within the organization rather than official SOP. It can be disruptive, revolutionary and radical. Examples could of digital innovation could be the first trials of internal custom app development for smart phones or software development groups using the Agile method, DevOps or Agile DevOps.
