How is being a CTO at a startup different from a CTO at a large company?

Top Answer : You have a much tighter budget.  The business is making pivots all the time, and you have to make sure the app stack and everything keeps pace with these pivots.  You have to deliver as soon as possible, as soon as the business demands it.  This requires that you use your resources, your engineers, wisely.   That is perhaps the biggest challenge to being a CTO at a smaller company, that things move so fast and IT and engineering must keep pace with business speed.   My previous employer was a larger company, and we would have 6 months, sometimes a year, to deploy.  But now, at a startup, we have a much tighter timeline to deliver.  Our sprints aren’t 2 weeks or 1 month away, but 1 week away.  If we can’t deliver in time, then we cannot go live, and we cannot analyze the data with A/B testing to determine if what we’ve done is the correct stuff or not.  That’s why I use agile methodologies.
