What domain name registrars and DNS hosting providers do you feel are the most reliable?  This may be a mundane question, but with the Register.com's 3rd outage in less than eight months, one may wonder...

I don’t believe any are completely reliable. However, Register.com has proven they are the least reliable and I’ll be moving the last domain sitting there to another registrar. I’ve already purchased secondary DNS services through DNSimple for that one to make it a bit more resilient.

Used DynDNS for many years and was very satisfied with the service. However, after some changes and acquisition by Oracle decided to move to another provider. I am sure the service is still good though - https://www.oracle.com/cloud/networking/dns/ Currently using https://www.name.com/ and also https://www.cloudflare.com/, so far so good.

If you are already using CloudFlare as your CDN, EndPoint Security, DNS etc then their Domain Registrar is definitely the way to go. Simple, Cheap and very reliable. I've moved domains to their free plan just to use their Registrar.
